Intravenous DexamethasoneYou decide to give some intravenous dexamethasone, and prescribe this, then site a cannula. During the procedure Jack becomes much more upset with severe stridor. After the cannula is taped down, you decide to examine him.
HR 165/min, RR 55/min, T 37.4 C, SaO2 85% in Oxygen. His chest recession is now moderate to severe and it is easy to hear his respiratory noise at rest. His chest remains clear and breath sounds are equal.
Because of his deterioration you call the team from paediatric intensive care who attend quickly. They review him and note also the normal radiograph. He is given an adrenaline nebulizer and taken to PICU. You hear later that he was intubated because of his breathing difficulty. |
